Make a referral
Anyone can make a referral to Options for Life, it can come from the participant themselves, a family member or carer, social workers, GPs, teachers, or any other person with an interest in the participant’s health and well-being.
Please download the referral form by clicking here, ensure all questions on the referral form are completed in full.

Complaints
There are times when you know that something is wrong and will want to get it sorted straight away. At Options for Life we will make it our priority to work with you to resolve the issue as quickly as possible.
Talk to a member of staff
If you have a concern about the support the person you care for is receiving from us, in the first instance, you can talk to your Support Team Manager or Head of Operations who will work with you to solve the issue.
